Key facts

  • One KwikPen holds 4 weekly doses of one strength: 2.4 mL in total, 0.6 mL per dose.
  • You prime before every weekly dose, then turn the dose knob until it stops. You do not count clicks.
  • After 4 doses the pen will not dial a full dose, even with liquid left. Never draw the rest into a syringe.
  • A new dose means a new pen in the new strength; one pen cannot give two different doses.

KwikPen strengths

The strength on the carton is the total in the pen, four doses’ worth. Each dose is the same 0.6 mL.

Setting a KwikPen dose

The Instructions for Use set out the same two steps every week:

  1. Prime. With a new pen needle on, turn the knob to the prime position (2 clicks), point the needle up, tap out air, then press and hold for a slow count of 5. A drop at the tip means the pen is primed.
  2. Dose. Turn the knob all the way until it stops. That is your full weekly dose. Inject, press the knob in until it stops and hold for a slow count of 5.
Do not count clicksThe dose is set by turning the knob until it stops, not by a number of clicks. Clicks only matter for the 2-click prime position.
Pen needles are not includedEach dose needs a new KwikPen-compatible pen needle, bought separately. Never share a pen, even with a new needle.

After four doses

The pen stops you dialling a full dose after the fourth weekly dose. Liquid left inside is expected: the label says to throw the pen away after 4 doses “even if there is medicine left in it”, and the Instructions for Use say not to transfer it into a syringe, which would give an incomplete, unknown dose.

Changing your KwikPen dose

Each pen is one strength. When your prescriber moves you up, for example from 5 to 7.5 mg after at least 4 weeks, you start a new 7.5 mg pen; you cannot dial a different dose on the old one. Finish the current pen’s four doses or follow your prescriber’s instruction on when to switch.

What a KwikPen costs

Through Lilly’s self-pay programme the KwikPen and the vial share one price tier: $299 for a month at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg and above, as long as each refill is completed within 45 days. One month is 28 days and one pen. The single-dose pen is not in this tier. Read 13 Oct 2026.

Frequently asked questions

How many doses are in a Zepbound KwikPen?

Four weekly doses of one strength, 0.6 mL each, 2.4 mL in total.

Do you count clicks on the Zepbound KwikPen?

No. Turn the dose knob until it stops; that is the full dose. Clicks only mark the 2-click prime position.

Why is there liquid left in my KwikPen after 4 doses?

It is expected. The pen locks after four doses and the label says to throw it away even if medicine is left. Do not draw the rest into a syringe.

Can I change my dose with the same KwikPen?

No. Each pen delivers one strength; a new dose means a new pen in that strength.
