Missed a tirzepatide dose
What the Zepbound and Mounjaro labels say to do, with a worked example for a Monday injection.
Key facts
- Take the missed dose as soon as possible within 4 days (96 hours).
- More than 4 days late: skip it and take the next dose on your usual day.
- Either way, then carry on once a week as before. Never take two doses to catch up.
- You can move your injection day as long as two doses are at least 3 days (72 hours) apart.
The 4-day rule
Both labels give the same instruction: if a dose is missed, take it as soon as possible within 4 days. If more than 4 days have passed, skip it and take the next dose on the regularly scheduled day.
Worked example: usual day Monday
| You remember on | What the label says |
|---|---|
| Tuesday | Take it now, then inject again next Monday |
| Wednesday | Take it now, then inject again next Monday |
| Thursday | Take it now, then inject again next Monday |
| Friday | Take it now, then inject again next Monday |
| Saturday | Skip it; inject next Monday as usual |
| Sunday | Skip it; inject next Monday as usual |
Counting from the time you would normally inject: up to 96 hours later you take it, after that you skip.
Changing your injection day
You can move to a different day of the week if you need to, as long as the time between two doses is at least 3 days (72 hours). Then keep the new day every week.

Frequently asked questions
What if I miss my tirzepatide dose by one day?
Take it as soon as you remember; you are within the 4-day window. Then take the next dose on your usual day.
What if I miss my dose by 5 days?
Skip it and take the next dose on your usual day. Do not take two doses close together.
Can I change the day I inject tirzepatide?
Yes, as long as two doses are at least 3 days (72 hours) apart.
I missed two weeks of tirzepatide. Can I restart at my usual dose?
The labels do not say. Ask your prescriber before restarting; some people are restarted at a lower dose to limit side effects.
