Switching from semaglutide to tirzepatide

Moving from Ozempic, Wegovy or compounded semaglutide to Zepbound, Mounjaro or compounded tirzepatide: what is set, and what your prescriber decides.

Key facts

  • There is no conversion table: neither the Zepbound nor the Mounjaro label describes a switch from semaglutide.
  • The labels start everyone on 2.5 mg weekly; any other starting dose is your prescriber’s decision.
  • Do not take both: the label advises against combining tirzepatide with any GLP-1 medicine.
  • In SURMOUNT-5, average weight loss over 72 weeks was 20.2% on tirzepatide and 13.7% on semaglutide.

What the labels say about switching

Nothing direct. The Zepbound and Mounjaro labels give one schedule for everyone, starting at 2.5 mg, and do not mention coming from semaglutide. What they do say is that tirzepatide should not be combined with other tirzepatide products or with any GLP-1 receptor agonist3, which includes semaglutide. So the switch is a stop and a start, never an overlap.

No milligram equivalenceSemaglutide and tirzepatide doses are not interchangeable. 1 mg of semaglutide is not “equal” to any dose of tirzepatide, and no table that claims otherwise comes from either label.

Which tirzepatide dose to start on

The labels start at 2.5 mg, the dose meant to let the stomach adjust. Someone coming from semaglutide has already been through one escalation, and some prescribers start higher for that reason; that is a clinical judgement outside the label, made for the individual. Every later step follows the usual rule: 2.5 mg at a time, at least 4 weeks apart.

See 2.5 mg and 5 mg for the units each takes from every kind of vial.

Timing the switch

The label sets no gapNeither label says how long to wait after the last semaglutide dose. Both medicines are taken once a week, and the timing of the first tirzepatide dose is something to agree with your prescriber before you stop semaglutide.

Expect stomach side effects to return for a few weeks after the switch and after each later increase; they are most common during escalation.

Why people switch

Most often for more weight loss. In SURMOUNT-5, 751 adults with obesity and no diabetes took tirzepatide or semaglutide at the highest dose they tolerated; after 72 weeks they had lost 20.2% and 13.7% on average1. In type 2 diabetes, SURPASS-2 found every tirzepatide dose lowered HbA1c more than semaglutide 1 mg2. Price, coverage and supply are the other common reasons.

Frequently asked questions

What dose of tirzepatide do you start on after semaglutide?

The labels start everyone at 2.5 mg weekly and do not describe switching from semaglutide. Some prescribers start higher for people already used to a GLP-1; that is an individual clinical decision.

Can you take semaglutide and tirzepatide together?

No. The tirzepatide label advises against combining it with any GLP-1 receptor agonist, which includes semaglutide.

How long after semaglutide can you start tirzepatide?

The labels set no waiting period. Agree the timing of your first tirzepatide dose with your prescriber before stopping semaglutide.

Will I lose more weight on tirzepatide than on semaglutide?

In SURMOUNT-5, average weight loss over 72 weeks was 20.2% on tirzepatide and 13.7% on semaglutide. Individual results vary widely.
