How long tirzepatide takes to work
What happens in the first day, the first month and the first year, from the label and the trials.
Key facts
- Blood levels peak about 24 hours after an injection (range 8 to 72 hours).
- With weekly doses, levels reach a steady state after 4 weeks.
- The full dose is reached slowly: at least 20 weeks to step up to 15 mg.
- Trial weight loss was measured over 72 weeks; most of it builds over months, not weeks.
In the body: day one to week four
After an injection, tirzepatide reaches its highest level in the blood after about 24 hours, anywhere from 8 to 72 hours. Its half-life is about 5 days, which is why it is taken once a week, and with weekly doses the level in the blood settles into a steady state after 4 weeks1. The label notes that its slowing of stomach emptying is strongest after the first dose and lessens with later doses.
Weight loss: months, not weeks
Everyone starts on 2.5 mg, a dose meant to begin treatment rather than to work fully, and steps up every 4 weeks or more. In SURMOUNT-1 the dose was raised over up to 20 weeks and results were measured at 72 weeks: 15.0% average weight loss at 5 mg, 19.5% at 10 mg and 20.9% at 15 mg2.
Blood sugar
In type 2 diabetes, SURPASS-2 measured HbA1c after 40 weeks: it fell by 2.01 to 2.30 percentage points depending on dose3. HbA1c reflects about three months of blood sugar, so it changes more slowly than daily readings.
Why it can feel slow
- The first 4 weeks are on a starting dose not meant to be the effective one.
- Each increase needs at least 4 weeks on the dose before.
- Side effects can mean staying longer on a dose, which is allowed.
Whether and when to move up is a decision for your prescriber. See the dosing chart for the full schedule.
Each dose step settles in about four weeks
The label’s 4-week steady state is also why the schedule waits at least 4 weeks on each dose: by the time a step is due, the current dose has reached a stable level in the blood. Stomach side effects are most common in the weeks after an increase and usually ease as the dose settles.
Sleep apnoea
In the SURMOUNT-OSA trials the effect on breathing during sleep was measured at 52 weeks: interruptions fell by 25 to 29 an hour on tirzepatide, from about 50, against about 5 on placebo4.
Children with type 2 diabetes
The Mounjaro trial in children aged 10 and over ran a 30-week placebo-controlled period, followed by a 22-week extension, on 5 or 10 mg.
What to keep track of
- Your weight once a week, at the same time of day, rather than daily.
- Side effects after each increase, and how long they last.
- If you have diabetes, your usual glucose readings, and HbA1c every few months.
- The date of each dose step, so you know when the next one is possible.
How long to stay on it
Zepbound is approved for weight reduction and its long-term maintenance, and the trial evidence points the same way.
SURMOUNT-4 tested what happens after stopping. 670 adults lost 20.9% on tirzepatide over a 36-week lead-in; those then switched to placebo regained 14.0% of their weight by week 88, while those who continued lost a further 5.5%. Overall, the continued group was 25.3% below its starting weight and the placebo group 9.9%.
The effect on the stomach
Tirzepatide slows how fast the stomach empties, which is part of why people feel full sooner. The label measured it with paracetamol (acetaminophen): after a first 5 mg dose its peak level fell by 55% and arrived an hour later, but by week 6 on 15 mg there was no meaningful effect. The slowing is strongest at the start and fades with continued use.

Frequently asked questions
Is it normal to feel no change in the first month?
The first 4 weeks are on 2.5 mg, a starting dose that is not approved as a maintenance dose. The trial results describe months of treatment at higher doses.
Does a higher dose work faster?
Blood levels rise in proportion to the dose, and the trials found more weight loss at higher doses, but each dose still takes about 4 weeks to reach a steady level.
How long does Zepbound stay in your system?
Its half-life is about 5 days. By the usual rule that a medicine is almost cleared after about five half-lives, that is roughly a month after the last dose.
When do Zepbound side effects start?
Often in the first days after the first dose or an increase; the label notes that the slowing of stomach emptying is strongest after the first dose and lessens with later doses.
How long does it take to see results on Mounjaro?
Blood sugar control was measured at 40 weeks in SURPASS-2. Daily readings change sooner, and the dose is increased only if more control is needed.
How long does tirzepatide take to work?
It peaks in the blood about 24 hours after a dose and reaches a steady level after 4 weeks of weekly doses. Trial weight loss was measured over 72 weeks.
How long does tirzepatide stay in your system?
Its half-life is about 5 days, so it takes several weeks after the last dose to clear.
Why am I not losing weight on 2.5 mg?
2.5 mg is a starting dose, used for 4 weeks to let the body adjust, and is not approved as a maintenance dose.
When does tirzepatide lower blood sugar?
HbA1c was measured at 40 weeks in SURPASS-2, where it fell 2.01 to 2.30 points. Daily readings change sooner.
